Autodebit Authorization

Please complete the authorization agreement form for your monthly dues to be automatically deducted from your bank account. Complete the form and attach a copy of a voided check. Please remit the form and attached voided check to:

Allow 30 days for the autodebit to be activated. Therefore, your autodebit payments will not take effect until you receive notification from the Union Bank of California. The autodebits are deducted from the account around the 8th of each month.
